Pay Range: $43,459 – $47,982 annually

Position Details: This is a full-time position; however, they will be considered in temporary status for the first 12 months due to potential changes in the job duties.

The SAGE Treatment Navigator-Mental Health Professional is a member of a multi-disciplinary responsible for providing behavioral health services (mental health, substance abuse disorders, and physical health) to individuals enrolled in Medicaid. This position works in coordination with SAGE case managers to help clients that are having a difficulty getting to regularly scheduled outpatient behavioral health services with their assigned case manager, or have dropped off services due to hospitalization, chronic disease, jailed, recently have experienced a crisis. Services are provided to both homeless and housed clients that seek services and are enrolled or willing to be enrolled in SAGE outpatient behavioral health services.

Outreach CM are based out of the 515 3 RD Ave., 216 James Street, Wall Street (Hobson Clinic) outpatient behavioral health centers. Many services will be provided out in the community, Permanent Supportive Housing (PHS), shelters or other service centers as assigned. This position requires a high degree of coordination & collaboration with other DESC programs as well as outside agencies. Interested individuals should have experience with community and behavioral health case management, resource referral, and work well with individuals that are the most vulnerable in the community.

MAJOR DUTIES AND RESPONSIBILITIES

Case Management:
* Provide variety of case management activity focused on re-engaging with SAGE case manager with all SAGE outpatient Treatment Navigation Teams; 515 3 rd Ave, 216 James Street, 222 Wall Street (soon to be Hobson) Clinics.
* Provide and prioritize crisis intervention with clients as needed. This may include de-escalation, going to ERs, and/or making referrals to County Designated Mental Health Professionals.
* Assist SAGE Treatment Navigator case managers and medical assistants with training on basic behavioral health case management and documentation.
* Assist as needed with conducting intake evaluations, mental health risk assessments, develop support and treatment plans in order to re-enroll clients back into services as needed, and with the greatest client participation possible.
* Collaborate with SAGE multidisciplinary team in coordinating clients’ support and treatment plans, until they can meet regularly with their primary SAGE CM. This includes providing and/or coordinating the following: survival services with a focus on access to food, shelter and clothing; fastest possible acquisition and stabilization of housing; acute and ongoing medical care, psychiatric treatment, financial assistance, criminal justice system coordination, substance abuse treatment.
* Advocate for clients’ access to community resources and services, ensuring that clients’ needs are met and rights maintained; consult and collaborate with community providers to ensure continuity of care.
* Develop and maintain strong collaborative relationships with DESC staff, and other service and resource organizations to ensure full continuity of care for clients.
Outreach & Engagement:
* Provide outreach and engagement services in coordination with client’s SAGE multidisciplinary team. Outreach and Re-engagement efforts take place on the streets, in day centers and shelters, PSH properties, jails, hospitals, or crisis centers/ER. Efforts are balanced between service engagement need and client’s continuity of care needs.

* Interact with general client population, intervene in crisis situations with individual clients in drop-in day center, new client walk-in’s, unscheduled walk-in’s with enrolled clients.
* Develop and maintain cooperative relationships with current programs providing services for homeless and mentally ill persons. Encourage development of new services as needed.
* Provide continuity of care for any client that is challenged to navigate traditional behavioral and physical health care systems.
* Participate in education of service community regarding the needs of homeless clients with mental illnesses/co-occurring disorders, techniques of engagement and service provision.
Other duties & responsibilities:
* Prepare and participate in psychiatric consultation, program meetings and in-service trainings; participate in clinical reviews and case conferences for clients enrolled in SAGE services that may not have follow through with primary case manager or treatment team.
* Comply with the agency’s clinical accountability policies and procedures; maintain timely and complete clinical records; participate in quality assurance reviews when assigned.
* Comply with applicable program research and evaluation procedures.
* Other duties as assigned.
MINIMUM QUALIFICATIONS:
* Interest or experience in working with clients who are difficult to engage and refer to traditional programs.
* Ability to meet Washington state requirements for registration as a Mental Health Professional.
* Washington Driver’s License and insurable driving record.
* Knowledge of Recovery focused, strengths-based work in behavioral health.
* Knowledge of Harm Reduction strategies.
* Ability to work effectively with clients displaying a wide range of unpleasant and/or bizarre behaviors.
* Ability to communicate and work effectively with staff from various backgrounds.
* Subscribe to the philosophy of cooperation and continuity across programs and of consideration and respect for clients.
EDUCATION AND EXPERIENCE:

Mental Health Professional:
* Ability to obtain Washington Department of Health minimum credential as an Agency Affiliated Counselor or higher
* Master’s degree in a social work, counseling, psychology, marriage and family counseling, or other relevant behavioral science.
* Meet criteria as a Mental Health Professional as defined in RCW 71.05, WAC 388-865-0400.
