Pay Range: $22.48 – $24.82 per hour
Shift: Office Day

DESC is a nationally recognized innovator in homeless services. Our original, and still foundational service, is our Emergency Shelter Program. Since we opened in 1979, DESC has operated emergency shelter for the most vulnerable men and women experiencing homelessness in our community. Unlike traditional shelter systems, DESC incorporates harm reduction principles into all of our work, meaning we ensure we are providing shelter to those who are most in need, not the most willing to simply meet program requirements.
The Rapid Rehousing (RRH) Case Manager works with DESC shelter clients one-on-one to create comprehensive plans to secure and maintain housing that will end their homelessness. The RRH Case Manager assesses housing barriers of potential RRH participants to address the immediate, practical barriers to moving into housing. This position provides focused services and referrals that assist clients to obtain or maintain income and successful tenancy in the rental market. Services are delivered in the shelter and clients’ homes.
MAJOR DUTIES AND RESPONSIBILITIES
  • Identify and outreach shelter clients that may be appropriate for Rapid Rehousing (RRH) Services
  • Orient prospective participants about RRH Services, and determine eligibility
  • Complete Rapid Re-Housing applications with clients and collect eligibility documentation
  • Create Housing Stability Plans with participants and update them on a monthly basis
  • Closely coordinate with other DESC staff to ensure continuity of RRH services for participants
  • Conduct monthly home visits and status checks of eligibility documentation
  • Introduce newly housed clients to their local resources and services
  • Develop plans and follow-up strategies to help participants use or develop skills to be successful tenants
  • Consult and collaborate with participants’ care teams to ensure effective planning for clients
  • Ensure services are documented and reported in accordance with agency and regulatory standards, as well as applicable grant requirements
  • Become familiar with local employment, housing and other resources.
  • Cooperate with other programs and agencies in the social service system to provide good networking of client services.
  • Participate in staff meetings and training sessions.
  • Other duties as assigned.
MINIMUM QUALIFICATIONS
  • Ability to meet state requirements for registration as a counselor.
  • Experience working with the homeless, economically disadvantaged, mentally ill or chemically affected.
  • Ability to communicate and work effectively with clients and staff from various backgrounds.
  • Ability to work effectively with clients displaying a wide range of unpleasant and/or bizarre behavior.
  • Commitment to the philosophy of cooperation and continuity across agency programs and to the philosophy of consideration and respect for clients.
  • Bilingual in English/Spanish is highly preferred.
EDUCATION AND EXPERIENCE
  1. Relevant Bachelor’s degree in social work, psychology, or related behavioral science, or
  2. Bachelor’s degree in any field and 1 year of paid work experience in social services, or
  3. Relevant associates degree and 1 year of relevant paid work experience, or
  4. Any Associates degree and 1 year of highly relevant paid work experience, or
  5. A combination of 1 year of highly relevant paid work experience and formal training in clinical and case management skills to demonstrate an ability to successfully perform the job.
PREFERRED QUALIFICATIONS:
  • A relevant master’s degree in social work, psychology or counseling is highly preferred.
