Outpatient Services Outreach Case Manager

As part of a multi-disciplinary treatment team, you will provide targeted outreach to youth, young adults, and caregivers in the outpatient programs, who are struggling to engage. Utilizing outreach and case management techniques, you will support clients in understanding and accessing appropriate treatment and health promotion resources.
Key Ares of Responsibility:
Engage clients in various on campus and community settings including schools, satellite offices, homes, housing programs, etc.
Encourage and utilize individual and family strengths and capabilities.
Develop and implement strategies, along with multi-disciplinary team members, to provide outreach services to hard-to-reach youth, young adults, and caregivers currently enrolled in one of our outpatient programs.
Develop knowledge in peer support, community resources, education, housing and food resources aimed at helping clients connect to resources related to managing physical and behavioral health issues.
Utilize a wide variety of case management and outreach interventions to meet clients’ needs and engage them to reconnect to services.
Maintain case records in accordance with licensing, accreditation and agency requirements.
Essential Job Requirements:
Acceptance and ability to work with individuals and families with a variety of lifestyles, behaviors, cultural and spiritual practices with the ability to apply a social justice and equity lens to all aspects of this work.
Interest and ability in working with adolescents, particularly those from marginalized groups, with mental health or substance use or co-occurring disorders.
Accredited bachelor’s degree in behavioral science and two years relevant experience, preferably in social or vocational services with young people, or a master’s degree in mental health related field.
Ability to work offsite at a community partner
